Describe, as best as you can, your type of clientele in terms of what you see as their need for sex services. (Not just the 'need to get off' but why they hire help; what they are missing in their lives.)
The men I talk to have a kink they want to express. Be it crossdressing or spanking or humiliation they want to express that with someone. Many have a kink or fetish that can't be shared with their partner. Often the assumption is the wife or girlfriend would freak out if she found out about his true fantasies. This of course isn't true in all cases. Some married men call me for the thrill of being with another woman. Since it's phone sex most don't see it as cheating so it's guilt free. The single men I talk to don't have anyone to play out their fantasies with so they call me.
Do you try to help with those needs, even if the client hasn't specifically asked or even acknowledged this need? If so, how? If not, why not?
Often yes. I treat this fantasies as normal. I don't judge them. Just talking to an accepting person helps a lot. If a man's kink isn't too radical I try to suggest how he can broach the subject with his partner.

I talk to a lot of crossdressers. I've helped them buy clothes and taught them how to put on makeup. I've given them some websites to buy clothes made for men who crossdress. Often crossdressers will carry shame with their fetish. My treating it as normal and exploring their kink they come to accept themselves as they are.
Years ago I talked to a man with a spanking fetish. He had told his wife who freaked out, hated him for it and ultimately divorced him. He was terrified of returning to the dating scene. We talked about his divorce and moved to playing some with spanking fantasies. I encouraged him to befriend fellow kinksters. I thought meeting like minded people would help him out a lot.
